Transitions
The transition to adulthood can be a particularly challenging time for some young people, who are particularly vulnerable and require additional support. Learning from Safeguarding Adult Reviews has highlighted how ineffective transition planning can contribute to young people "slipping through the net" or "facing a cliff edge" with tragic consequences. Recent Safeguarding Adult Reviews reflect this learning, with some common themes identified, including the young person being at high risk of exploitation, alcohol and substance use with no identified support needs under the Care Act.
This protocol has been developed jointly with the Gateshead Safeguarding Children's Partnership (GSCP) and aims to promote robust transitions between children and adult services.
